Visage is set inside a large house where terrible things have happened. Players need to find fragments of the history of the house, with each gradually revealing what is behind the dark history of the place, while witnessing firsthand the "remnants" of each death left behind. The game is scheduled to be divided into four chapters. Currently, only the first two chapters are playable as part of the early access period.

Players walk around the house from a first-person perspective, are capable of manipulative objects and doors and also need to manage defined resources, such as lighter fluid and spare lamps, as they explore the house trying to discover the truth. The multiple closed doors and a search for the only keys that open are a central part of the game's progression. The house is designed as a semi-open world, with great features exploitable to players, including areas not relevant to the chapter in question. The ghosts will manipulate some aspects of the house while the player explores, being either programmed events or random phenomena for example: light bulbs lighting, doors closing themselves, shaking objects and the like.

There are three ways to die in the game: being killed by dangerous elements scattered around the scenes, being killed by the various ghosts and demons that inhabit the house or losing your sanity. Sanity, whose operation is similar to game mechanics like Amnesia: The Dark Descent, is reduced whenever the player witnesses enemies or paranormal events, and passively if the player remains in darkness for a long time. The lower the player's health, the ghosts of the house become more active the probability of the player experiencing random paranormal phenomena or facing hostile ghosts. Sanity can be restored by taking pills scattered around the house or staying in well-lit areas. The game has no health indicator so if the player is captured by the ghosts he dies instantly.
