In 2019, LeFay returned to game development, co-founding OnceLost Games alongside another Bethesda veteran, Ted Peterson. In a heartfelt statement, Ted Peterson, CEO of OnceLost Games, lamented the situation of his friend and colleague: "Julian has been bravely battling cancer. His doctors have informed us that his time with us is limited, and we are preparing to say goodbye to a true industry legend."

Despite the illness, LeFay's dedication to The Wayward Realms and the community never wavered. He worked tirelessly to ensure his vision for the "Great RPG" would survive. Peterson stated that LeFay "has complete faith in the team's ability to bring the game he envisioned to life for this amazing community."
